Aging Well Eldercare cares deeply about its clients and staff. This Client Rights and Responsibilities outlines the standards of service we will provide to our clients, as well as our expectations regarding clients' treatment and duties to our staff.

At Aging Well Eldercare (AWE), we are dedicated to providing you and your family with compassionate, individualized support as you or your loved one navigate the challenges of aging or disability. Our mission is to deliver the highest quality of care without discrimination, embracing all aspects of your identity, including age, race, ethnicity, religion, gender, and sexual orientation.

Outlined below are your rights and responsibilities while engaging in services with AWE:

Your Rights:

  1. To be treated with respect and dignity by AWE staff, regardless of age, race, religion, gender identity, sexual orientation, or any other protected status.

  2. To receive care management support that is tailored towards individual needs, preferences, and goals.

  3. To have personal and medical information kept confidential and only shared with consent, except as required by law.

  4. To participate in decisions about care and to terminate services verbally or in writing to the office.

  5. To be informed about the rate of services and notified of any changes.

  6. To voice concerns or complaints about the services provided and our billing practices, without fear of retaliation, and to have issues addressed promptly and fairly. Any concerns or complaints should be directed to the AWE office.

Your Responsibilities:

  1. To treat AWE staff with respect and dignity, regardless of their age, race, religion, gender identity, sexual orientation, or other protected status.

  2. To provide accurate and complete information about your demographics, health, needs, and preferences to ensure appropriate care is received.

  3. To communicate any changes in condition or concerns to your care management team in a timely manner.

  4. Our staff members are not allowed to accept any gifts valued at more than $25, although we always welcome notes of appreciation regarding our staff and the support they provide to our clients and their families.
