Care Manager–Nurse (Part Time – 20 to 30 hours/week)
We are actively seeking an RN interested in joining the rewarding and expanding field of aging life care management. We are looking for a dedicated, independent, creative and experienced nursing professionals with excellent interpersonal skills and a background in aging and health care. RN degree required and CCM or CMC certification a plus. Experience in geriatrics, dementia, home health or hospice care and/or mental health strongly preferred. The aging life care manager position is flexible with a minimum of 20 hours per week up to 30 hours per week. We visit clients around the D.C. metropolitan area, primarily in Montgomery County, Northwest D.C., Frederick County, and Northern Prince George’s County. You will need to be a self-starter, able to work independently and communicate well with clients, families and other professionals. Pay commensurate with experience.
